Clinician discussion brief
Clinician discussion copy / Sample 001 / fictional edition 1.2
Context reviewed 29 October 2026. No person was sequenced, no laboratory call was verified, and no professional reviewed a real patient in this demonstration.
Purpose and scope
This is an educational preview of the information a member might bring to a clinician or pharmacist. It is not a medical record, a clinical test report, a referral or a treatment recommendation. Every genotype and finding assigned to the sample is invented.
Medication review
Simulated call / M01
CYP2C19 *17/*17
Ultrarapid metabolizer
In CPIC’s PPI guideline, this diplotype maps to the ultrarapid phenotype. For selected PPIs, that phenotype may be associated with lower exposure and reduced treatment success. The ultrarapid PPI prescribing recommendation is optional; this sample supplies no dose instructions.
- Would a clinically confirmed CYP2C19 result be relevant to an actual PPI indication or treatment response?
- What assay scope, allele definitions and confirmation would be needed before using the result?
- How do other medicines, interactions, history and response change the interpretation?
- What follow-up would be appropriate if a prescribing decision is made?
No medication or supplement list, indication, symptoms or prior response is supplied. Do not start, stop or change a medicine or dose using this sample.

Interpretation and test limits
- H01 / SIM-V204 — VUS. Invented unresolved variant identifier. It cannot establish a diagnosis and should not direct a care change by itself.
- H02 / SIM-R12 — no-call. A synthetic unresolved region. The illustrative 31.8× average does not imply uniform coverage.
- H03 / scope-limited negative. No reportable finding in the invented scope; this is not an all-clear result.
- H04 / PRS withheld. No model, calibration or clinical inputs were applied. No percentile or absolute risk is available.
Questions: Does clinical or family history justify further evaluation independently? What additional testing, if any, would address the original question? MedlinePlus: interpreting genetic results.
Family planning
The teaching model assigns one reproductive parent Aa for a single hypothetical recessive condition. The other parent is untested. All numerical probabilities are withheld in the initial model. A real carrier discussion requires condition-specific clinical testing and counseling; AA in the teaching model is not equivalent to a negative screening result.
Question: What screen, detection limits and residual risk are relevant to both reproductive parents? ACOG: carrier screening.
Version history
Fictional finding SIM-V102 moves from uncertain in v1.1 to likely benign in v1.2. This is an invented reclassification to show how a report can record reduced concern. It is not a real laboratory event or evidence of an active monitoring service.
Question: What evidence changed, who reviewed it, and is there any implication for earlier care? NCI: genetic testing results and reclassification.
Required for a clinical handoff
- Identity and consent verified in a secure clinical process.
- Laboratory report, method, reference build, allele definitions, scope and quality limitations.
- Confirmed findings and their classification, source evidence and review date.
- Clinical and family history, medicines, symptoms and the reason testing was sought.
This file collects none of that information. No genome upload or email sending is connected to this file. The separate prelaunch account contains no genetic data.
Proposed lifetime interpretation updates mean updates for the lifetime of the Whole DNA service, within its supported analysis scope. New sequencing, laboratory assays and clinical appointments are separate. Final cadence, retention and exit arrangements must be confirmed before ordering opens.
